Fill out this form with your personal information and;

• Check off 10 Year Abstract (SDA)
• Check off Confirmation Letter and specify “Operator History Letter” and ALSO indicate the reason you need the letter
• COVID Interim Policy*: You do not need to have the form notarized, just leave that part of the form blank. The piece of Photo ID also does not need to be notarized.

2. We will check the information and send you a credit card link for $90.00 for the abstract & confirmation letter and registered mail* return costs (*government policy). 
3. We will send the government your documentation on your behalf
4. The Government of Alberta takes 6 weeks to process the confirmation letter. Once we receive it, we will mail the letter via registered mail at the client’s expense (included in $90 fee).
